Day Habilitation Program

Day Hab. Drivers & Assistant Drivers

- Requirements: Valid N.Y.S. driver's license - minimum 3 years of experience with license, & Highschool Diploma


DSP/Habilitation Specialist 

Positions available Part-time with hours ranging from 12, 16, 20, 24 hours per week.

Evenings hours: average of 4 hours per shift.

Duties will include:

Will work directly with individuals in program activities, both in their homes and in the

community.

Assist the individual in skills development by implementing consumer specific and general

programs.

Maintain daily Community-Habilitation bi-weekly progress reports, data sheets, and other

required documentation.

Act as a role model for consumer by appropriate behavior and offering assistance as indicated

in Individual Service Plans.

Attend and participate in staff meetings and In-service training.

Requirements:

High school diploma/GED

Good oral and written communication skills.

Day Habilitation Program

Title: Direct Support Professional –DSP

Bilingual – English/Spanish a plus

Duties and responsibilities of DSP will include:

 Transport of individuals - Pick Up and Drop Off from home and to assigned sites in

community

 Provide instruction, supervision and support to the individuals.

 Prepare written and oral progress reports, maintain records etc.

Minimum Qualification: High School Diploma/GED.

Valid Driver’s license and able to drive a 15 passenger van

Skills / experience

Experience working with intellectually disabled persons a plus.

Ability to work independently, and travel to assigned locations to implement services, etc.

SCIP, CPR, First Aid training a plus.
