Medicaid Service Coordination
Adult Resources Center, Inc.’s Medicaid Service Coordinators assist individuals with developmental disabilities in gaining access to supports and services that are appropriate to their respective needs. Our qualified Service Coordinators work in partnership with the individual, advocates and other service providers to set personal goals in enriching the individual lives. Most of the activities of the Service Coordinator fall within four main categories in providing efficient services, namely: Individual Service Plan, Linkage/ Referral, Advocacy, Follow ups and monitoring ongoing services. Our Service Coordinators also assist individuals in attaining the highest quality of life as well as to live independently and productively as possible in the community. Person Centered planning is utilized in developing, implementing and maintaining the Individualized Service Plan.
Individuals are also linked with Homes and Community Based Waiver Services (HCBS) such as Respite, Supportive Employment, Community Habilitation (Self Direct), Prevocational Program, Day Habilitation, Recreational, and Residential Habilitation Services (IRA). In addition, Service Coordinators direct individuals to Article 16 and 18 clinical services such as Medical, Dental, Psychological/ Psychosocial evaluations, Ophthalmologist, Podiatry, Counseling and Home Care in addressing the needs of our Individuals.
